New Patients
Dr Lokesh Yagnik welcomes new patients. Please download this form and email it to us or bring along with you to your first appointment. Alternatively arrive 10 minutes prior to your appointment to complete a form.
Service
This is a private clinic and fees are charged for service provided. Some services are covered in part or full by medicare and some by your health care provider. The clinic phone (0432807009) is answered from 8am till 4pm Mon-Fri Only. SMSes will generally not be answered.
